Pamper Party Consent Form!

 I, the undersigned, confirm that I am the parent or legal guardian of the child named above and hereby give permission for them to participate in a children’s Pamper Party hosted by Laura Michelle Makeup.

 I understand that the event includes a variety of pampering activities such as (but not limited to): facials, face masks, exfoliators, hair styling & makeup.

 I acknowledge and accept that although every reasonable effort is made to ensure the safety and well being of all children, participation in any activity carries inherent risks. By signing this form, I voluntarily assume all risks associated with my child’s participation and agree to release, waive, and hold harmless Laura Michelle Makeup from any and all liability for injury, loss, or damage, including personal or property loss, that may arise during or as a result of the event. 

 Allergy & Sensitivity Disclaimer

I understand that Laura Michelle Make up uses a range of beauty products and treatments that may contain common allergens or skin irritants. I agree to disclose any known allergies or sensitivities related to my child prior to the party and will notify staff of any updates. While reasonable precautions are taken, I acknowledge that Laura Michelle Makeup cannot guarantee a completely allergen free environment, and I agree not to hold the business or its staff liable for any allergic reactions or adverse effects.

By signing this agreement I confirm that I have fully read and understood the terms of this waiver.
